Affiliates are expected to submit a report utilizing this template by following instructions on this page.

If the template does not exist on the page

  1. Open up the page you wish to create
  2. Select "Edit"
  3. Start typing {{ to open up the Template Inserter
  4. Enter for "Affiliate Health Criteria Form" in the search box
  5. Select "Affiliate Health Criteria Form" from the drop-down in the search box
  6. Enter the affiliate information into the template, following the prompts and examples
  7. When the form is complete, select "Insert"
  8. Save the page

If the template already exists on the page

  1. Select "Edit"
  2. Click on the template
