Terminology

I doubt "perpetrator" is the most suitable term in this context. Maybe "sanctioned individual"? Nemo 12:33, 30 July 2019 (UTC)

About the content of the request:

1. first describe the encountered problems;
2. make abstraction of all specific cases;
3. then propose a review or an internal audit.

About the sequence in the review process:

1. first resolve the pending problems;
2. talk selectively to a few top level board members;
3. then formally request a review or internal audit;
4. then wait for the result of the review/audit;
5. then possibly amend some procedures.
