The purpose of this form is to verify in writing that the grantee's information has not changed since a grant was last received, or to confirm any changes. This is so that grantees don't have to provide information they may have already provided in the past. This form has a version for organizations and a version for groups and individuals. The grants administrator will choose the correct form for your grant, but you may read more about the information requested in each form here.

These fields apply to all grantees

Please answer "yes" or "no" in response to these questions. If you select "yes," the form may request more information from you about the change.

  • List of people with access to your account or who will sign the grant agreement: please indicate here if anyone has been added or removed from your account. For each new individual, you will be asked to provide the name, address, birthdate, and a photo identification document. If there have been no changes, please select "no."
  • Bank information: please indicate here if you have changed your bank account or if information such as your bank account number, SWIFT code, or bank branch has changed. If there have been no changes, please select "no."
  • Other changes: please indicate if there have been other changes we should know about before processing your grant. If there have been no changes you need to inform us of, please select "no."
